Contact form and Donate idea?

Does anyone recomment an effective wordpress plugin that will create a contact form on a page of my blog?

I've had a lot of questions emailed or sent to me in other ways about people's individual pets & health concerns, some of which have been quite complicated.

I'd like to make an "Ask Ferox" sort of page with a contact form and paypal donate button. I'm not sure if it will work or not, but I figure it must be worth a try.

Anyone recommend a contact form?

The contact form I used was this one: WordPress Contact Form with Spam Protection | The Marketing Technology Blog and that's the only one of 3 that I managed to get to work.

I used a form plugin by Dagon Design. I've had great success with it.
That said, yours looks good, too!

CForms is the best contact form plugin I've found. The 2nd best is the one from Dragon Design...

I've done both as well... the button on the main template of every page, while the contact form is in the Contact section. I wrote my own form and you can easily do it too - I even put up the source code for it on my site. If you're interested in making your own, let me know if you need assistance.

It's definitely worth a try Ferox but I'm not sure how successful donation schemes for blogs are.
