I picked up a php/mysql application for surveys the other day. Looks easy enough to install...

I'm looking for one that will keep track of the information and will give me graphs of the demographics from the questions I put in the survey.

I haven't had a chance to install it or play around so I can't really tell you much.

I have been looking at using social networks/media and qualitative market research...this app I found when trolling the aul resarch communities. So, if I find more I shall post them... this is the first one I am gonna play with

If you're looking to just get some basic information, SurveyMonkey is awesome.

It won't be hosted on your site, but the free version has basic reporting and has worked for everything I've ever needed to survey.
