What will happen to my blog when I update?

I used a wordpress blog for my client and updated the main image in the banner. I have been reading that if I update the wordpress version "things" will happen to the blog. What are these things? Will I lose the comments, etc?

Any suggestions for updating, avoiding losing anything, etc?
