Did you mean design your own theme completely? Nope, didn't do that, I did however migrate my blog from digg3 to my new theme and I consider that doing the design myself. I had to look up the theme, learn to adapt the CSS to fit my needs, etc. It was a lot of work for a non-techie momma like me

And that is good work indeed! It is a useful skill that if you get proficient at might make a good job skill. A decent CSS designer can make fairly good money.

I think it depends on what you are trying to accomplish. Some of the most popular blogs aren't spectacular in design and people visit for the content. If you want a very specific look and feel design away but if you want to use your blog as a means to convey your thoughts a template will probably do. That said, I will probably revamp the design of StuffRichPeopleLove.com in the next 12 months but again, I will probably use a predesign and then tweak it.

I taught myself css and designed my own blog. I've redesigned my blog twice. I create cartoons for all my blog posts so it was easy to use them as part of my design.

No, I didn't design my own. For my first blogs, I found a great site with some free templates and my latest blog, I paid a few $$ for the design.
